Accidente cerebrovascular y factores de riesgo. Estudio prospectivo controlado
Abstract
Objetivos: Comparar presencia de factores de riesgo entre pacientes con stroke (S) y controles (C), determinar predictores de stroke.
Material y métodos: Diseño prospectivo, observacional, longitudinal, analítico. Se incluyen pacientes de 18 o más años con primer stroke y controles con edad de±5 años con respecto al caso correspodiente e igual género y sin antecedentes de stroke. Muestreo consecutivo con 46 casos y 46 controles.
Resultados: Mujeres 58,69%. Stroke isquémico 86,95%. Edad 69,76±1,41, tabaquismo 35,86%, sedentarismo 84,78%, alto nivel de estrés en 30,43%, antecedentes familiares de enfermedad vascular 29,34%, nefropatía crónica 8,69%, en todos los casos pNS entre S y C. IMC fue 27,18±0,54,(S 28,64±0,85, C 25,72±0,63, p 0,006). Cardiopatía 77,17%, (S 89,13%, C 65,21%, p 0,006).Hipertensión 64,13% (S 89,13%, C 39,13%, p 0,00001). Diabetes 33,69% (S 45,65%, C 21,73%,p 0,02. Dislipemia 56,52 % (S 65,21 %, C 47,82 %, p 0,14). Mal control: 67,79 % de hipertensos (S 82,92%, C 33,33%, p 0,0005), 77,41% de diabéticos (pNS entre S y C) y el 88,46% de dislipémicos (pNS entre S y C). Predictores de S en análisis univariado: mayor IMC (p 0,01), hipertensión (p 0,0000), diabetes (p 0,01) y cardiopatía (0,0091). En multivariado: hipertensión (0,0001).
Conclusiones: En el análisis bivariado se comportaron como predictores de stroke: obesidad, hipertensión, diabetes y presencia de cardiopatía. En el análisis multivariado solo se comportó como predictor de stroke la hipertensión. La prevalencia y el mal control de hipertensión, diabetes y dislipemia fueron elevados

Hypertension: 64.13% (89.13% S, C 39.13%, p 0.00001). Diabetes: 33.69% (45.65% S, C 21.73%, p 0.02. Dyslipidemia: 56.52% (S 65.21%, C 47.82%, p 0.14). Poorly controlled: 67.79%, of hypertensive patients (82.92% S, C 33.33%, p 0.0005), 77.41% of diabetics (pNS between S and C) and 88.46% of dyslipidemia (pNS between S and C). S predictors in univariate analysis: higher BMI (p 0.01), hypertension (p 0.0000), diabetes (p 0.01 ) and heart disease (0.0091). In multivariate analysis: hypertension (0.0001).
Conclusions: In bivariate analysis behaved as predictors of stroke: obesity, hypertension, diabetes and heart disease presence. In multivariate analysis only hypertension was a predictor of stroke. The prevalence and poor control of hypertension, diabetes and dyslipidemia were higher.
