EL VALOR DEL ÍNDICE CRONOTRÓPICO PARA ESTADIFICAR Y ESTRATIFICAR EL RIESGO EN LA ENFERMEDAD DE CHAGAS

Resumen

Introducción. Ciertos marcadores de disfunción autonómica podrían proporcionar información pronóstica adicional para estratifcar el riesgo en la enfermedad de Chagas (ECh). Objetivos. 1) Describir las características ergométricas iniciales y el perfl de riesgo cardiovascular de una población con ECh, 2) comparar la utilidad del cálculo del índice cronotrópico (ICr) sobre el porcentaje de la frecuencia cardíaca (FC) máxima alcanzada, 3) describir la importancia de la recuperación del tono vagal. Materiales y métodos. Estudio descriptivo y comparativo de corte transversal. Pacientes con ECh que realizaron una prueba ergométrica entre agosto de 2015 y agosto de 2019. Se excluyeron los portadores de marcapasos y aquellos pacientes con enfermedad coronaria, miocardiopatía dilatada o diabetes sacarina o medicados con betabloqueantes o amiodarona. Resultados. Los pacientes con ICr bajo no presentaron bradicardia sinusal en el registro basal (71 l. p. m. vs. 76 l. p. m.; p = 0,07). El ICr se mantuvo bajo en las personas con ECh sin patología demostrada (Kuschnir 0; ICr 0,78 vs. Kuschnir 1; ICr 0,67). Se utilizó la prueba de Student para el análisis estadístico de variables continuas. El valor de p <0,05 se consideró signifcativo. Conclusiones. El cálculo del índice cronotrópico podría incrementar el número de pacientes con pruebas insufcientes y, a la larga, modifcar la estadifcación de esta población. Se sugiere desestimar la bradicardia basal como manifestación aislada de disautonomía.

THE VALUE OF CHRONOTROPIC INDEX FOR STAGING AND STRATIFYING RISK IN CHAGAS DISEASE
Abstract
Introduction.
Certain markers of autonomic dysfunction could provide additional prognostic information for risk stratifcation in Chagas disease (ChD). Objectives. 1) Describing initial ergometric characteristics and cardiovascular (CV) risk profle in a ChD population, 2) comparing the usefulness of the calculation of the chronotropic index (ChI) on the percentage of the maximum heart rate (HR) achieved; 3) describing the importance of recovering vagal tone. Materials and methods. Descriptive and comparative cross-sectional study. Patients with ChD who underwent an ergometry between August 2015 and August 2019. Patients with pacemakers, coronary heart disease, dilated cardiomyopathy, diabetes mellitus, beta-blockers and/or amiodarone were excluded. Results. Patients with low ChI without sinus bradycardia at baseline (71 bpm vs 76 bpm; p = 0.07). ChI remains low in people with ChD without proven pathology (Kuschnir 0) (Kuschnir 0; ChI 0.78 vs Kuschnir 1; ChI 0.67). The t Student test was used for the statistical analysis of continuous variables. A value of p <0.05 was considered signifcant. Conclusions. Determining ChI could contribute to detecting greater numbers of patients with insuffcient tests, and eventually modifying the staging of this population. The study suggests that basal bradycardia should be ruled out as an isolated manifestation of dysautonomy.
