MENINGITIS ASÉPTICA INDUCIDA POR TRIMETOPRIMA/SULFAMETOXAZOL EN UNA MUJER JOVEN: A PROPÓSITO DE UN CASO

Resumen

Una mujer de 40 años, internada con un diagnóstico de celulitis en la pantorrilla izquierda, era tratada con trimetoprima/sulfametoxazol (TMP/SMX). Dos horas después de su primera dosis, de 800/160 mg, presentó cefalea, febre, vómitos, somnolencia, fotofobia y rigidez nucal. Se le realizó una tomografía simple de cráneo, con resultado normal. Se le efectuó una punción lumbar; esta reveló pleocitosis de 400 por mm3 con predominio de polimorfonucleares, hiperproteinorraquia de 89 mg/dl y glucorraquia normal. Se cambió el antibiótico por clindamicina, debido a la sospecha de meningitis aséptica como reacción adversa del TMP/SMX, y se agregó ceftriaxona ante el cuadro de meningitis; esto se suspendió al tercer día por el resultado estéril del cultivo del líquido cefalorraquídeo (LCR). Posteriormente, la paciente mencionó haber tenido un episodio de reacción alérgica al TMP/SMX. El cuadro se resolvió por completo a los tres días de la suspensión de TMP/SMX.

ASEPTIC MENINGITIS INDUCED BY TRIMETHOPRIM-SULFAMETHOXAZOLE IN A YOUNG WOMAN: A CASE REPORT
Abstract
A 40-year-old woman, admitted for a diagnosis of cellulitis in the left calf, was treated with trimethoprim/ sulfamethoxazole (TMP/SMX). Two hours after her frst dose of 800/160 mg she presented headache, fever, vomiting, drowsiness, photophobia and nuchal rigidity. A simple tomography was performed with normal results. Lumbar puncture revealed a pleocytosis of 400 per mm3, predominantly polymorphonuclear, and hyperproteinorrachia of 89 mg/dl; glucose was normal. Antibiotic was changed to clindamycin due to the suspicion of aseptic meningitis as an adverse reaction of TMP/ SMX, and ceftriaxone was added for suspected meningitis, which was suspended on the third day due to sterile result of the culture of the cerebrospinal fluid (CSF). Subsequently, the patient mentioned having had an episode of allergic reaction to TMP/SMX. The disorder was completely resolved within three days of the suspension of TMP/SMX.
