TROMBOEMBOLISMO VENOSO EN PACIENTES CON CÁNCER

Resumen

Se evaluó la forma de presentación, evolución y complicaciones del tromboembolismo venoso (TEV) en pacientes con cáncer. Se incluyeron 89 pacientes, de los cuales 53 (59,6%) fueron mujeres. La mediana de edad fue 65 años (rango: 23-97). Veintiséis pacientes (29,2%) fueron diagnosticados y tratados de manera ambulatoria. Se diagnosticaron 65 trombosis venosas profundas (TVP) y 12 superfciales (TVS). Entre los 73 pacientes con trombosis de miembros, el 76,7% tenía la enfermedad en miembros inferiores y el 23,3%, en miembros superiores. Se diagnosticó tromboembolismo pulmonar (TEP) en 20 personas. En las extremidades inferiores, 91,1% fueron TVP y 8,9% fueron TVS, en tanto que fueron bilaterales 23,2%. En los miembros superiores, 58,8% fueron TVP y 41,2%, TVS. En los 63 pacientes hospitalizados, la mediana de la duración de la internación fue siete días (rango: 1-72 días). La mortalidad hospitalaria fue 20,6%. Durante el tratamiento, hubo siete sangrados mayores y cinco retrombosis. La mediana de la supervivencia fue 166 días. La probabilidad de supervivencia a los 30 días fue 0,80; a 60 días, 0,73; a 90 días, 0,59; a 180 días, 0,49, y a 365 días, 0,40. La supervivencia de los pacientes bajo tratamiento activo fue menor que la de aquellos que se hallaban en control. La supervivencia de los pacientes que desarrollaron TEV nosocomial fue menor que la de los pacientes que se encontraban bajo tratamiento ambulatorio al momento del
diagnóstico del TEV. No se observó diferencia en la mortalidad hospitalaria ni en la mediana de la supervivencia entre los pacientes con TEP y aquellos sin la enfermedad.

VENOUS THROMBOEMBOLISM IN CANCER PATIENTS
Abstract
A prospective, observational study was conducted to evaluate the clinical presentation, evolution and treatment complications of venous thromboembolism (VTE) in cancer patients. Eighty-nine patients were included; 53 (59.6%) were female. The median age was 65 years (range: 23-97). Twenty-six (29.2%) subjects were diagnosed and treated as outpatients. Sixty-fve deep vein thrombosis (DVT) and 12 superfcial vein thrombosis (SVT) were diagnosed. Of the 73 patients with extremity thrombosis, 76.7% had lower and 23.3%, upper limb thrombosis. Pulmonary embolism (PE) was diagnosed in 20 subjects. In lower extremities, 91.1% were DVT and 8.9%, SVT; of them, 23.2% were bilateral. In upper extremities, 58.8% were DVT and 41.2%, SVT. In the 63 admitted patients, the median hospital stay was 7 days (1-72). In-hospital mortality was 20.6%. Of them, 17.8% patients without PE and 27.8% with PE died (p = 0.4923). There were seven major bleedings and fve rethrombosis during treatment. The median survival was 166 days. The survival probability at 30 days was 0.80; at day 60, 0.73; at
day 90, 0.59; after 180 days, it was 0.49, and after 365 days, 0.40. The survival in patients under active treatment was lower than in patients under control. The survival in patients with nosocomial VTE was lower than in patients who were ambulatory at VTE diagnosis. No differences in in-hospital mortality or median survival were observed between patients with or without PE.
