EL DESGASTE PROFESIONAL CRÓNICO (BURNOUT) EN DOCENTES E INTEGRANTES DEL EQUIPO DE SALUD EN CIUDADES ARGENTINAS

Resumen

Introducción. La presente investigación se ha llevado a cabo con el objeto de cuantifcar el desgaste profesional crónico o burnout y sus características particulares en docentes de educaciónprimaria, media y universitaria, y también en integrantes del equipo de salud, en distintas etapas de la vida profesional, a lo largo de diferentes años y en distintas provincias del país. Material y método. Diseño observacional, transversal, analítico. Las investigaciones se desarrollaron entre los años 2003 y 2016; en algunas localidades los relevamientos de información se realizaron en más de una oportunidad y en diferentes años, siempre con la misma metodología. La recolección de datos se efectuó mediante encuestas de autorreporte en las que se aplicó la escala del Maslach Burnout Inventory (MBI) (1), instrumento validado para el idioma español. El muestreo fue no probabilístico. Para el análisis estadístico se utilizó el test t para las variables cuantitativas y el de chi cuadrado para las variables cualitativas; el análisis multivariado fue con regresión logística. Se usaron los programas SPSS 17.0 y Primer. Resultados. De 565 docentes (80,5% de enseñanza media y 19,4% universitarios) evaluados, presentaban burnout el 11,4% los de los niveles primario y medio, y el 8% del nivel universitario; el resultado fue signifcativamente mayor en los colegios de nivel medio y en los del interior de Córdoba. En el equipo de salud, el 77,1% fueron médicos (36,8% especialistas y 40,2% residentes) y el 22,9%, profesionales no médicos. El burnout fue variable, con medias de entre 26 y 44% en los médicos y del 19% en el resto de los profesionales. Conclusiones. El burnout fue mayor en el equipo de salud que en los docentes. En el equipo de salud fue superior en los médicos y signifcativamente en la etapa de residencia en hospitales escuelas. Parecería menor en el ámbito privado; la escala tiene aceptable confabilidad; no existe coincidencia entre las variables predictoras; sería menor en Córdoba que en Catamarca y La Rioja; parece signifcativamente creciente en el tiempo y se mantendría en un nivel cercano al 50%.

BURNOUT IN TEACHERS AND HEALTHCARE TEAM MEMBERS IN ARGENTINIAN CITIES
Abstract
Introduction.
This research has been undertaken with the aim of quantifying professional chronic burnout and its characteristics in primary and secondary education teachers, university teachers, and also health team members, in several stages of their professional life, over different years and in different provinces of Argentina. Material and methods. Cross-sectional, observational and analytical design. This research work was carried out between 2003 and 2016; in some places the surveys were conducted on more than one occasion and in different years, always with the same methodology. Data collection was done by means of self-reported surveys using the “Maslach Burnout Inventory” (MBI) (1), an instrument validated for the Spanish language. The sampling was not probabilistic. The statistical analysis was carried out using the t-test for quantitative variables, and the chi square test for qualitative variables; the multivariate analysis was done with logistic regression. SPSS 17.0 and First programs were used. Results. 565 teachers (80.5% secondary education teachers and 19.4% university teachers) were evaluated. 11.4% presented burnout in primary and secondary education,  and 8% at the university level. The result was signifcantly higher in secondary schools and in schools located inland in the province of Córdoba. In the health team, 77.1% were doctors (36.8% specialists and 40.2% residents), and 22.9%, non-medical professionals. The burnout was variable, with means of 26-44% in the doctors, and 19% in the other professionals. Conclusions. The burnout was higher in
the health team than in the teachers. In the health team, it was greater among doctors, and signifcantly
higher during residencies in teaching hospitals. It would seem to be minor in the private sector; the scale has acceptable reliability; there is no coincidence between predictors; it would be lower in the province of Córdoba than in Catamarca and La Rioja; it seems to increase in time signifcantly and it would remain at a level close to 50%.
