SISTEMA DE GUARDIAS NOCTURNAS EN BLOQUE: UN MODELO DE ORGANIZACIÓN DE GUARDIAS EN LA RESIDENCIA DE CLÍNICA MÉDICA: 17 AÑOS DE EXPERIENCIA
NIGHT FLOAT SYSTEM: AN ORGANIZATIONAL MODEL IN THE INTERNAL MEDICINE RESIDENCY PROGRAM OF AND EMERGENCY DEPARTMENT – 17 YEARS OF EXPERIENCE
Resumen
Desde el año 1999, se propuso en el Hospital Privado de Comunidad (Mar del Plata) reducir la cantidad de guardias de los residentes, con los objetivos de disminuir el agotamiento, reducir los errores médicos relacionados con el agotamiento mental, mejorar la capacidad de atención del residente y brindar una atención uniforme a todos los pacientes independientemente del horario de ingreso al hospital. A tal fin se implementó el sistema de “nochería”, que funciona con guardias nocturnas de 12 horas. La nochería se realiza dos veces en el año, con aproximadamente 25 días en cada rotación. Luego de estos 17 años de experiencia, se observa una franca mejoría en la calidad de vida del residente, en su capacidad de aprendizaje y en el aprovechamiento de la residencia, y una mejor calidad de atención a los pacientes.

Abstract
In 1999, the Hospital Privado de Comunidad (Private Community Hospital, Mar del Plata) decided to reduce the number of residents' guards, with the aim of decreasing exhaustion, diminishing medical errors related to mental exhaustion, improving the care capacity of the resident and providing uniform care for all patients regardless of the time of hospital admission. For this purpose, the "nochería" (night work) system was implemented, which works with 12-hour night guards. The work is done twice a year, with approximately 25 days in each rotation. After 17 years of experience, there has been clear improvement in the resident’s quality of life, in his capacity for learning and making good use of the
residency, and improvement in the quality of patient care.
