Estudio de las interconsultas solicitadas por los servicios quirúrgicos al servicio de medicina interna

Resumen

Introducción
Existe un incremento de interconsultas clínicas debido al envejecimiento poblacional y a nuevos procedimientos quirúrgicos aplicables en pacientes de mayor edad. Materiales y métodos
Se analizaron las características sociodemográficas y epidemiológicas de las interconsultas solicitadas en días laborables del periodo 01/06/2010-31/05/2013, por servicios quirúrgicos (cirugía general junto a vascular, urología, plástica, ORL, ginecología y tórax) al servicio de Medicina Interna, así como las diferencias con los no interconsultados.
Resultados
De 4.297 pacientes, se solicitaron 354 interconsultas (8,3 %) con una media de visita de 2,92. Ingresaron desde urgencias 167 (47,2 %) y programadas 107 (30,1 %). El 80,5 % las solicitó cirugía general, 6,5 % tórax y 5 % urología y ginecología. El 50,2 % fueron hombres. La edad media fue de 62,7 años (DE 18,5) y los hombres presentaron menor edad media (61,6; DE 17,9) (p = 0,2629). El 65 % fue dado de alta, el 25 % trasladado a otros servicios. Entre los no interconsultados (3.943) 21 % ingresaron desde la guardia y 73 % programados. El 89 % fue dado de alta y el 5,5 % trasladado a otros servicios. La mediana de estadía de los interconsultados fue mayor (10 vs 3 días; p = 0,0001) presentando además mayor edad media (62,7 vs. 54) (p = 0,000) y mortalidad (19,7 x 1.000 vs 1,5 x 1.000). Los motivos fueron evaluación general (26,3 %), nefrourológicas (12,4 %), cardiovasculares y síndrome febril (11,6 %).
Conclusiones
Los interconsultados presentaron mayor: edad media, mediana de estadía; más ingresos desde guardia; traslados a otros servicios y óbitos. El motivo más frecuente fue la evaluación general. Se demuestra la importancia de la interconsulta para identificar pacientes con mayor riesgo de presentar complicaciones clínicas.

STUDY OF INTERNAL MEDICINE CONSULTATIONS REQUESTED BY SURGICAL SERVICES

ABSTRACT
Introduction

There is an increment in the number of internal medicine consultations secondary to population aging and to the new procedures applicable to older patients.
Materials and methods:
We describe the characteristics of clinical consultations requested by surgical services (general surgery. vascular surgery. urology. plastic surgery. otorhinolaringology. gynecology and thoracic surgery) during the period 06/01/2010-05/31/2013. excluding weekends. We analize the epidemiological differences between these patients and those for whom no consultation was made.
Results:
From a total of 4297 patients. there were 354 consultations (8.3%). with a mean of 2.92. One hundred and sixty seven (47.2%) came from the emergency room and 107 (30.1%) were programmed. The 80.5% corresponded to general surgery. 6.5% to thoracic surgery and 5% to urology and gynecology. The 50.2% were men. The mean age was 62.7 years (SD 18.5). with men younger than women (mean 61.6; SD 17.9) (p=0.2629). A 65% were discharged from the ward while 25% were transferred to other services. From the patients without consultations (3 943). 21% came from the emergency room and 73% were programmed. 89% were discharged and 5.5% were transferred to other services. the median stay length was higher for patients with consultation (10 vs 3 days; p=0.0001). who also showed higher mean age (62.7 vs 54) (p=0.000) and higher mortality (19.7x1000 vs 1.5x1000). The reasons for internal medicine consultation were general assessment (26.3%). nephrourological causes (12.4%). cardiovascular causes and febrile syndrome (11.6%).
Conclusions:
The patients who required consultations presented higher mean age, higher median stay length, more admissions from the emergency room, more transfers to other services and higher death rate. The principal reason for clinical consultation was general assessment. We demonstrated the importance of internal medicine consultation in the identification of patients with higher risk of clinical complications.
