EL ACEITE DE GIRASOL Y LA LECHE DE VACA CUMPLEN UNA FUNCIÓN CLAVE EN EL DIAGNÓSTICO Y EL TRATAMIENTO DE LA ENFERMEDAD CELÍACA

Resumen

Introducción. Actualmente, el tratamiento de la enfermedad celíaca (EC) consiste en la exclusión de trigo, cebada, avena y centeno de la dieta. Sin embargo, el aceite de girasol y la leche de vaca pueden tener una acción negativa al incrementar la respuesta inflamatoria clínica e inmunológica. Objetivos. Establecer la función de los aceites y la leche de vaca en el desencadenamiento o la mitigación de la EC para optimizar su diagnóstico y su tratamiento. Material y métodos. Estudio prospectivo, de tipo ensayo clínico, con pacientes pediátricos con síntomas y serología positivos para EC que recibieron una dieta de exclusión de leche de vaca y aceite de girasol, con inclusión de aceite de oliva extravirgen y gluten, a los cuales se realizó un seguimiento clínico y serológico durante un año. Resultados. Los diez pacientes evaluados revirtieron síntomas de EC y negativizaron anticuerpos de alta especificidad sin excluir el gluten de la dieta. Conclusiones. El protocolo que se utilizó para este trabajo permitió revertir los síntomas de la enfermedad y provocó la negativización de los anticuerpos en sangre pese a que los pacientes continuaron con el consumo de gluten. Mostrar una serie de pacientes que revirtieron síntomas y anticuerpos con una especificidad superior al 90% plantea la necesidad de estandarizar la dieta previa a la biopsia, con gluten, sin aceite de girasol y sin leche de vaca. Esto influye en el actual protocolo de diagnóstico y tratamiento de EC.

SUNFLOWER OIL AND COW’S MILK HAVE A KEY ROLE IN THE DIAGNOSIS AND
TREATMENT OF CELIAC DISEASE

Abstract
Introduction. Currently, the treatment of celiac disease (CD) consists in excluding wheat, barley, oats, and rye from the diet. However, sunflower oil and cow’s milk can play a negative role in increasing the clinical and immunological inflammatory response. Objectives. To establish the role of the oils and the cow’s milk in triggering or mitigating CD in order to optimize its diagnosis and treatment. Material and methods. Prospective study, of clinical trial type, over pediatric patients with symptoms and positive serology for CD, who received a diet excluding cow’s milk and sunflower oil, including extra virgin olive oil and gluten, point at which they were followed up clinically and serologically for one year. Results. The 10 patients evaluated reversed symptoms of CD, and negativized antibodies of high specificity, without excluding gluten from the diet. Conclusions. The protocol used for this work allowed to revert the disease symptoms and caused the negativization of the antibodies in blood, even though the patients continued with the gluten consumption. Showing a series of patients that reversed symptoms and antibodies that have a specificity greater than 90% raises the need to standardize the diet prior to biopsy, with gluten, without sunflower oil and without cow’s milk. This influences the current protocol for diagnosis and treatment of CD.
