CARCINOMA DUCTAL IN SITU: GENERALIDADES, DIAGNÓSTICO, TRATAMIENTO Y CONTROVERSIAS

Resumen

El carcinoma ductal in situ (CDIS) constituye, en lugar de una sola entidad, un grupo heterogéneo de lesiones con diferente potencial de malignidad y se lo considera un verdadero precursor del cáncer. El tratamiento óptimo del CDIS de la mama es uno de los desafíos más importantes en los padecimientos mamarios que afrontan los clínicos en la actualidad. Se ha identificado mutaciones del gen p53 en 7-37% del carcinoma intraductal. En algunos casos, la mutación de este gen suele acompañarse de un grado nuclear más alto. La sobreexpresión de la proteína HER2 es una característica común del CDIS. El 15-20% de los casos de CDIS son positivos para HER2. Del 90% de los carcinomas que se diagnostican por la presencia de microcalcificaciones radiológicas, el 80% de ellas son lesiones in situ. El CDIS no tiene potencial de mortalidad, en teoría, por lo que la finalidad del tratamiento es reducir la posibilidad de recidivas como un carcinoma infiltrante, que ocurren en el 50% de los casos de estas recurrencias. La mastectomía es el tratamiento más efectivo disponible para el CDIS si nuestro objetivo es, simplemente, prevenir la recurrencia local. La mastectomía cura al 95-99% de las pacientes, pero a expensas de una importante morbilidad física y psicológica, por lo cual estaría indicada la reconstrucción mamaria inmediata. La tendencia actual es preservar la mama, luego del análisis de las distintas formas de presentación de estos tumores.

CARCINOMA DUCTAL IN SITU: GENERALITIES, DIAGNOSIS, TREATMENT AND CONTROVERSIES

Abstract 
Ductal carcinoma in situ (DCIS) constitutes a heterogeneous group of lesions instead of being a single entity, with different potential for malignancy and is considered a true precursor of cancer. The optimal treatment of ductal carcinoma in situ of the breast is one of the most important challenges in the mammary sufferings that clinicians face today. Mutations of p53 have been identified in 7 to 37% of intraductal carcinoma. In some cases, the mutation of the p53 gene is usually accompanied by a higher nuclear grade. Overexpression of HER2 is a common feature of ductal carcinoma in situ. 15-20% of DCIS are HER2-positive. 90% of the carcinomas are diagnosed by the presence of radiological microcalcifications; 80% of them are "in situ" lesions. The DCIS does not have mortality potential, in theory, so the purpose of treatment is to reduce the possibility of recurrence as infiltrating carcinoma, which occurs in 50% of these recurrences. Mastectomy is the most effective treatment available for DCIS if our goal is simply to prevent local recurrence. The mastectomy cures 95-99% of the patients, but at the expense of an important physical and psychological morbidity. The immediate breast reconstruction would be indicated given the current tendency to preserve the breast, according to the analysis of the different forms of presentation of these tumors.
